You may register with the practice if you live within the catchment area. When registering please bring your NHS medical card to the practice and those of any other persons registering with you. If you do not have a medical card, you will be asked to complete an Application to Register form. All patients over the age of sixteen will also be asked to complete a short questionnaire, which will provide us with details of your past and current medical history. This is confidential and is used to help us to give you the best possible care until we receive your medical records.
If you move out of the practice area, you will need to register with a new practice as soon as you move.
